Call (860) 560-5848. Referrals accepted from doctors, hospitals, VNAs, and town social services. Assessment interview required.
What to expect
Referrals are accepted from doctors, hospitals, VNAs, and town social service departments. An interview and/or home visit is required to assess need. Once approved, we will begin meal delivery within two days of receiving the application.
